Finally, bring any information available regarding the medical treatment which you have received as a result of your injuries.  You will need to list any medical providers whom you have seen, including their contact information, and dates of service.  Also provide any discharge papers or bills which you have received.  Medical providers would include, hospitals, doctors, and ambulance services if necessary.
