Living Will
A living will communicates your health care choices if you are determined to be in a terminal condition or permanently unconscious
state. This document also includes an Organ Donation election form stating your wishes regarding organ donation.
Health care Power of Attorney
A Health Care Power of Attorney allows you to name a competent adult to make health care decisions if you are unable to do so.
i.e. Alzheimer's or Dementia. The Living Will takes precedence over the Health Care Power of Attorney should you fall into terminal
condition or permanently unconscious state.
